• Serve as the primary point of contact for assigned clients and families • Conduct onboarding calls and care assessments to establish initial care plans • Review caregiver visit notes and feedback to identify care issues, gaps, or changes • Coach and support caregivers on client-specific care needs and expectations • Address client concerns and resolve issues related to care quality or consistency • Proactively check in with clients to ensure satisfaction and adjust care as needed • Collaborate closely with Scheduling, Staffing, and Operations teams to ensure care plans are executable • Document all care plan updates, communications, and decisions clearly and consistently
