• Conduct telephonic outreach to members with diabetes to assess needs and close gaps in care • Provide education and support to promote member engagement and self-management • Identify and address social determinants of health impacting care • Refer members to appropriate care management programs and community resources • Collaborate with providers and interdisciplinary teams to coordinate care • Apply nursing processes in compliance with company and regulatory standards • Document interactions and care plans accurately in electronic medical records • Consult with supervisors to resolve challenges and improve member outcomes
