Job Description

• report directly to the Manager, Care Management • ensure optimal continuity of care for patients • support patients transitioned in/out of services • navigate healthcare options and Medicare coverage • handle customer issues and complaints • make clinical level of care determinations • follow up and track referral and admission outcomes • participate in Company's mandatory training • provide education regarding Home Health, Hospice, and Palliative Care Services • assist in admissions process

Job Requirements

Associates Degree required
BSN preferred
compact state RN license required
at least 3 years post-acute experience
nursing background across post-acute care
current CPR certification
knowledge of home health, hospice, and palliative care services
excellent analytical and problem-solving skills
must read, write, and speak fluent English
